Participants of the Personalized Stroke Prevention Program have been identified by their primary healthcare provider for being at increased risk for stroke. The team aims to support these participants with medication adherence, home blood monitoring and health – related behavior changes on an individual basis. This time will be utilized to take blood pressure and review individual progress.
