Why Participate?
Managing the day-to-day challenges of diabetes can be a struggle for individuals and families living with this condition. This online video conferencing class covers topics such as monitoring blood sugar, medications, meal planning tips, basic nutrition and how to avoid the complications of diabetes.
Who Can Participate?
You can join the program if you are 18 years or older and have been told by your health care provider that you have diabetes.
Program Goals
Learn to manage diabetes with a focus on: healthy eating, being active, taking medications, healthy coping, reducing risks, problem solving, monitoring.
