The Frazier Family Coalition offers diabetes self-management education classes recognized by the Association of Diabetes Education and Care Specialist.
The 2-class Series Includes:
Class 1: Overview of Diabetes
Class 2: Healthy Eating & Being Active
Individual counseling session with a dietitian
We are offering three sessions to choose from:
Mastering Diabetes is a 2-day class that helps you take control and manage diabetes.
Class 1: Overview of Diabetes.
Class 2: Healthy Eating and Staying Active
Free nutrition counseling available.
- April 9th (class 1) and 16th (class 2) / 6:00-8:00pm
- May 7th (class 1) and 14th (class 2) / 6:00-8:00pm
- June 4th (class 1) and 11th (class 2) / 6:00-8:00pm
Location: This is a virtual class, delivered via Zoom
Why Participate?
Managing the day-to-day challenges of diabetes can be a struggle for individuals and families living with this condition. This online video conferencing class covers topics such as monitoring blood sugar, medications, meal planning tips, basic nutrition and how to avoid the complications of diabetes.
Who Can Participate?
You can join the program if you are 18 years or older and have been told by your health care provider that you have diabetes.
Program Goals
Learn to manage diabetes with a focus on: healthy eating, being active, taking medications, healthy coping, reducing risks, problem solving, monitoring.
