Our High Blood Pressure Control and Healthy Living Program is a virtual 7 session program that focuses on the American Heart Association Life’s Essential 8: Eat better, Be more active, Quit tobacco, Get healthy sleep, Manage weight, Control weight, Manage blood sugar, and Manage blood pressure.
This Program Includes:
- Coaching with Nurse Navigator
- Nutritionist, and/or Pharmacist
- Two In-Person Group Activities
Who Can Participate?
We invite you to join this program if you are 18 years or older and have been informed by your health provider that you have hypertension or desire to live a heart-healthy life.
We are offering three sessions to choose from:
Start Dates:
- Tuesdays, October 3rd, 10am – 11am
- Tuesdays, October 3rd, 7pm – 8pm
- Wednesday, October 4th, 7pm – 8pm
